This form is to request a PERMANENT change to your child's bus transportation to and/or from school. Please contact your child's school directly for the occasional, one-time only transportation change.

Please keep in mind:

If your child has moved, you MUST provide proof of the new residency to the school district registrar ( 708-647-2310) before this form can be processed.
Requested changes take between 24-48 hours once this form is submitted. The school district's transportation coordinator will inform you of effective date.
